- Usage information: how you interact with the CareCoord platform, including pages visited and features used

- Provide and manage your care coordination services
- Communicate with your care coordinator
- Store and organize your documents and records
- Send appointment reminders and service updates
- Process billing and subscription management

Your information is stored securely in our database with access controls in place. Documents uploaded to the portal are stored securely. While we take reasonable steps to protect your information, no system is completely secure. You are responsible for keeping your login credentials confidential.

- You and authorized CareCoord staff have access to your portal account and uploaded documents
- We do not sell your personal or health information to third parties
- We may share your information with your healthcare providers when necessary to coordinate your care, or as required by law
5. Document Storage and HIPAA Notice
CareCoord stores your uploaded documents (medical records, insurance cards, billing records) in our secure portal.
6. Cookies
We use essential session cookies to keep you logged into the portal. We do not use advertising or tracking cookies.
7. Your Rights
You have the right to:
- Access the information stored in your portal account
- Request deletion of your account and associated data by contacting us
