1. About CareCoord

CareCoord provides healthcare navigation and care coordination services, including appointment scheduling assistance, medical records organization, insurance paperwork support, provider communication, and related administrative support. We are not a medical practice, healthcare provider, insurance company, or government agency.

2. Not Medical Advice

CareCoord does not provide medical advice, diagnosis, treatment, nursing services, or clinical care of any kind. Information and support provided through our platform and services are for care coordination and administrative assistance only. Always consult a licensed healthcare professional regarding medical decisions, treatment options, or health conditions.

3. No Guarantee of Outcomes

CareCoord makes reasonable efforts to assist clients with healthcare navigation and coordination, but we cannot guarantee appointment availability, treatment outcomes, insurance approvals, provider responsiveness, or healthcare provider actions. The healthcare system is independent of CareCoord, and all medical decisions remain between you and your providers.

4. Independent Service

CareCoord acts as an independent care coordination service and is not affiliated with, endorsed by, or employed by any specific healthcare provider, hospital, insurance company, physician group, or government agency unless expressly stated in writing.

5. Payment Terms

• Basic plan: $49/month, billed monthly
• Support plan: $99/month, billed monthly
• Concierge plan: $199/month, billed monthly
• Payments are processed securely through Stripe. All charges are final unless otherwise required by law.
• Subscription auto-renews each month until cancelled.
• To cancel: log into your account portal and cancel from the billing settings, or contact us at the email below.

6. Cancellation Policy

You may cancel your subscription at any time. Cancellations take effect at the end of the current billing cycle. No refunds are provided for partial months, but you will retain access until the end of your paid period.

7. Limitation of Liability

CareCoord shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services or inability to access healthcare services, appointments, or insurance benefits. Our total liability shall not exceed the fees paid by you to CareCoord in the 12 months preceding the claim.

8. Client Responsibilities

By using CareCoord, you agree to:

• Provide accurate and complete information about your healthcare needs
• Maintain the confidentiality of your account credentials
• Notify us of any changes to your contact or insurance information
• Use the portal responsibly and not for any illegal or unauthorized purpose

9. Changes to Terms

CareCoord reserves the right to modify these terms at any time. Changes will be posted on this page with an updated "Last updated" date. Continued use of the service after changes constitutes acceptance of the new terms.
